Provides coding expertise to Agencies assigned for ICD 9-10 coding for compliance. Assesses OASIS, medication profile, and H/P to ensure accurate coding for each patient and ensures that all applicable federal laws are followed.Responsibilities includeProvides Home Health coding for assigned agencies.  Applies knowledge of medical terminology, disease processes, and pharmacology. Demonstrates tested quality and integrity skills. Able to make independent decisions regarding accurate ICD-9-10-CM.

 Assigns and sequences ICD-9-CM/CPT/E/M codes to diagnoses and procedures for documented information.  Assures the final diagnoses and operative procedures as stated by the physician are valid and complete. Abstracts all necessary information from health records to identify secondary complications and co-morbid conditions.  Abstracts all necessary information and assigns codes (ICD-9, CPT & E/M ), which most accurately describe each documented diagnosis, surgical procedure and special therapy or procedure according to established guidelines. Know the rules and guidelines for coding based on payer guidelines with special attention to Medicare and Medicaid coding regulations. Determines the final diagnoses and procedures stated by the physician or other health care providers are valid and complete. Analyzes provider documentation to assure the appropriate Evaluation & Management (E & M) levels are assigned using the correct CPT code. Maintain familiarity with medical terminology and anatomy and physiology. Maintain coding credentials and continuing education credits (CEUs). Understand and pratice HIPAA privacy and security rules and regulations.

Protect the security of medical records to ensure that confidentiality is maintained.Process patient admission and discharge documents.Review records for completeness, accuracy and compliance with regulations.Compile and maintain patients' medical records to document condition and treatment and to provide data for research or cost control and care improvement efforts.Enter data, such as demographic characteristics, history and extent of disease, diagnostic procedures and treatment into computer.Release information to persons and agencies according to regulations. Plan, develop, maintain and operate a variety of health record indexes and storage and retrieval systems to collect, classify, store and analyze information.Manage the department and supervise clerical workers, directing and controlling activities of personnel in the medical records department.Identify, compile, abstract and code patient data, using standard classification systems.Resolve/clarify codes and diagnoses with conflicting, missing, or unclear information by consulting with doctors or others to get additional information and by participating in the coding team's regular meetings.Train medical records staff.Assign the patient to one of several hundred "diagnosis-related groups", or DRGs, using appropriate computer software.Process and prepare government forms.Contact discharged patients, their families, and physicians to maintain registry with follow-up information, such as quality of life and length of survival of cancer patients.Prepare statistical reports, narrative reports and graphic presentations of information such as tumor registry data for use by hospital staff, researchers, and other users. Assigns and sequences ICD-10 codes to diagnoses and procedures for documented information. Attends to annual inspection of Philippine Health Insurance for approval of hospital’s accreditation as required by the Department of health.

Handles Guess line for T/shirts, polo shirts, caps, socks and bottoms.Works with the R and D designer regarding the color matching of print and embroidery designs of T/shirts, Caps and Socks.Prepares bill of materials for season design of GUESS clothes line for Adult department.Prepares job order for production department.Prepares purchase order for each season need of accessories.Coordinates with the supplier for final approval of shirt and caps print design.Checks delivery of finished goods against the purchase order submitted to suppliers.
