Graduate Research Associate in the Center for Health Sciences Interprofessional Education, Research and Practice within the University of Washington School of Nursing Department of Biobehavioral Nursing and Health Systems.Managing grants focused on improving the interprofessional collaborative practice within the University of Washington Medicine's Regional Heart Center. The goal of the grants is to create an integrated, collaborative learning system at the University of Washington Regional Heart Center which connects disciplines, promotes teamwork, fosters mutual understanding, strengthens research, and advances health for individuals and populations. As patients with advanced heart failure (AHF) require direct care from multiple services, there is a great need to improve communication and coordination of care to deliver value as required by an ACO.
