CurrentPromoted to Behavioral Care Manager Lead:- Collaborating with the Director of Operations and BCM Team to ensure productivity and streamline processes-Working cross-functionally as point of contact with MAS, Providers, and Engineering to improve patient experience, and increase engagement- Created project management system for BCM Team as well as escalation process for identify patient needs/concerns- Supporting scaling start-up needs and contributing to onboarding processes- Implemented clinical safety protocol to improve escalation processes and assess client's risk BCM Role:- Serving as primary point of contact for a caseload of up to 150 families- Acting as liaison while engaging in the Collaborative Care Model, to coordinate care for families between internal care team (consisting of therapists, practitioners, nurses, and coaches) and families’ local primary care to deliver accessible telehealth care- Conducting needs assessments to triage patients to appropriate level of care with support of clinical staff- Performing tasks to support ongoing patient care, such as conducting intakes, scheduling regular one-on-one check ins, identifying community resources, and coordinating referrals
