Supported Community Living Specialist (Cdbg)
CurrentThe Supported Community Living Specialist will provide Habilitation services and Community Support Services (CSS) to assigned clients within their homes and community settings. implement group and individual services to clients in accordance with the client’s service plan. The Specialist is responsible for completing all treatment planning paperwork and activities for CSS clients. Provide appropriate interventions to teach clients skills involving activities of daily living, community integration, functional, social, advocacy, and skills to promote physical and emotional health. Documentation of services and progress toward goals. Contributes to the interdisciplinary team process and makes recommendations. Completes and maintains all CDBG documentation and related tasks. Provide appropriate interventions to teach clients skills involving activities of daily living, community integration, functional, social, advocacy, and skills to promote physical and emotional health.Responsible for completing all elements of treatment planning for CSS clients including but not limited to social histories, assessments, treatment plans, release of information, etc. and keep all records up to date and in compliance with applicable regulationsFacilitates client group learning and support activities as assigned in order to meet the needs of the client as well as client interests, and goals as defined by the interdisciplinary teamMaintain a caseload of assigned clients, initiating and scheduling client contacts as often as necessary to meet the client’s needs as well as providing intervention in crisis situationsEffectively work with persons with chronic mental illness in their home and community settings to maintain and/or increase skills Accurately document service delivery to meet billing standardsMeet productivity standards for the position by performing direct service delivery with persons individually and in group settingsTransports clients as needed for various reasons
