Performs on-site and remote case management and conducts interview with veterans and their families throughout the continuum of care. Monitors and reviews program requirements to ensure veterans who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems get the needed support. Clinical interventions and duties include a variety of modalities to include face to face visits, telephone, and video modalities.
