-Patient care coordination work including acquiring prior-authorization for medical tests (DatScan, Pet Scan, etc.), scheduling appointments, and dictating referrals for physical therapy, occupational therapy, speech therapy, home health aides and home safety assessments-Administers cognitive tests such as the mini mental status exam and the Montreal cognitive assessment (MoCA) to provide referral to the Alzheimer and Dementia Program-Connects patients with community resources such as Access-a- ride, meals on wheels, parking permit for people with disabilities, etc.-Works within Parkinson’s and Movement Disorder Program to facilitate weekly movement classes of 10-15 patients diagnosed with Parkinson’s disease-Composes letters of medical necessity and statements of condition-Assists with Intake and Referral phone line management for American Parkinson’s Disease Association (APDA)- Develops and maintains resource guide for patients and caregivers-Helps coordinate and participates in community activities sponsored by the hospital such as the Parkinson's Unity Walk, Brain Awareness Week, Annual Parkinson's Symposium, etc.
