Telehealth In The Home Project ManagerFlinders University Mar 2013 - Feb 2015Adelaide Area, AustraliaThis project recruited 162 participants receiving in-home telehealth services for palliative care, aged care and rehabilitation for the elderly in order to investigate the effect of online support provided to the homes and primary residences of patients and carers in the Southern Adelaide and Fleurieu Region. The support was based on a team approach integrating the patient and their carers, general practitioners, other primary health care providers, aged care facilities and specialist services. The project was led by Flinders University, in partnership with the SA Rehabilitation Statewide Clinical Network, the SA Older People Clinical Network and the SA Palliative Care Clinical Network.The project improved access to health services and the wellbeing of some of the most vulnerable people in our local communities. The previous home visiting model required extensive travel time for the clinician, limiting the number of patients seen each day. Evaluation of the healthcare outcomes and the options to sustain Telehealth in the Home services shows outcomes include a better quality of healthcare for the ageing and those requiring palliative care at home and efficiency gains in the health system. -
