The MD Culture Change Coalition was a collaborative organization, devoted to promoting person-centered care principles and practice in long term care settings in Maryland. The formal coalition activities are now dormant, but continued education of of these practices can be found at www.theconsumervoice.org and www.pioneernetwork.net.
