Discharge Planner
Current- Primary focus is on discharging patients from a skilled nursing facility to their homes or assisted living facilities. Keeping patients and families fully informed of all pertinent information regarding discharge.
- Work closely with insurance, home health and durable medical equipment companies to ensure patients are provided with all needed home health services and DME prior to discharge (including IV’s, PICC line meds, PEG tube.

- Maintain close contact and schedules with insurance companies, families and clinicians to ensure easy transition to Hospice, long term care or discharge home from facility.
- Work closely with Social Worker, Nursing, Business Office and Rehab Director in order to keep accurate discharge schedule and all necessary communications regarding all patient information via PCC. Keep accurate.
- Represent the company daily by providing excellent customer service to patients and their families. Communicate with patients and their families weekly and sometimes daily to ensure client’s satisfaction with services.
