Provide all necessary documentation to above mentioned in a timely manner.• Maintain close contact and schedules with insurance companies, families and clinicians to ensure easy transition to Hospice, long term care or discharge home from facility.• Work closely with Social Worker, Nursing, Business Office and Rehab Director in order to keep accurate discharge schedule and all necessary communications regarding all patient information via PCC. Keep accurate records, documentation, reports pertaining to all discharges.• Represent the company daily by providing excellent customer service to patients and their families.
