Provides residential mental health services to adults with a mental illness diagnosis co-occurring with chronic medical condition(s), focusing on serving clients that utilize ASL or gestural communication as a primary communication modality. Coordinates and/or completes the preparation of new charts, and logs all day-to-day occurrences, outcome charts, and other documentation to ensure accurate documentation of relevant behaviors and symptoms. Assists with coordination, access to, and administration of medication, health care appointments, behavioral interventions, and other support services as needed, determined by the individual’s care team. Supports development of individuals’ independent living skills. Assists program MHP and supervisor with 245d forms, paperwork, data processing, financial responsibilities, and other duties as needed. Develops and completes menu planning, food orders, food organization for the program, as well as ensures implementation of diets specific and appropriate to supported individuals’ needs. Independently developed and established program specific guidelines regarding menu planning and implementation thereof within 245d licensing requirements.

Provides proactive, empowering, and inclusive support, guidance, and supervision for deaf & developmentally disabled individuals. Assists with self-care, menu planning and preparing meals. Coordinates and/or completes the preparation of new charts, and logs all day-to-day occurrences, outcome charts, and other documentation to ensure accuracy and thorough completion. Schedules and accompanies individuals to medical appointments. Administers medication, monitors prescription renewal, and works with medical equipment. Assists with training new staff and works alongside the individual's guardians, families, case managers, and other team members.

Works with victim/survivors of sexual assault/domestic violence (DVSA) to plan for safety, find resources, improve their financial situation through job preparation and applying for benefits, searching for housing, and discovering ways to cope and heal. Provides advocacy and assistance in navigating through the judicial/civil court systems and federal/ state systems to provide non-governmental based case management supports for Orders for Protection, divorce proceedings, SSI/SSDI benefits, state and county services and benefits, and criminal proceedings. Provides support and de-escalation in crisis situations, and assists victim/ survivors to exit emergent danger in favor of shelters or other safe spaces.

Answers inbound calls via videophone (primarily video relay service calls) from members, clients and facilities. Verifies healthcare plan eligibility and coordinates non-emergent transportation and/or paratransit services utilizing transportation management systems, in order to meet contract service levels. Systems utilized were custom built in house programs or software built for AS400 systems. Provides top-quality customer service experience to a diverse customer population. Schedules trips in the most cost effective manner.

Assisted in developing an internet database of molecular information gathered from molecular calculations for internal company use with the goal of employees utilizing the information to help design new improved products in reduced amount of time. Summarized patents and helped create a website for the database to be hosted on. Utilized molecular modeling and computational chemistry tools to compute Quantitative Structure-Activity Relationships (QSAR) models, polymer chemistry models, solubility parameter models, and surface and interfacial energy models to calculate work of adhesion. Helped optimize and streamline initial development and reformulations of various projects in the Corporate Research Materials Laboratories (CRML) as well as other divisions throughout the company.

Utilized molecular modeling and computational chemistry software tools to build and optimize monomer and molecule structures. Carried out molecular dynamics simulation methodologies in two different software packages to compute molecular solubility parameters and to optimize/investigate structures of complexes of different kinds of molecules. The results of these simulations contributed to CRML projects in the areas of renewable materials and new product development within 3M’s Infection Prevention Division.

Assisted in developing a new manufacturing process to coat membranes to filter viruses that would be cost-effective but still efficient. Tested samples of membranes and conducted trials of different materials/coatings. Assisted with new product development within the Biological Materials Group in CRML.
