This role includes: - Conducting psychosocial needs assessments, and implementing clinical intervention. - Maintaining effective collaborative communication with healthcare providers related to the patient’s psychosocial and psychiatric needs. - Helping patients understand their diagnosis and treatment plan. Examining coping mechanisms and providing emotional support. - Providing feedback and input during board meetings to ensure patient-centered care is being practiced in accordance with the Commission on Cancer guidelines. - Providing grief, bereavement, stress management and self-care support to patients, families, and caregivers. - Being part of the interdisciplinary team for Palliative Care consults.

In this role, I was a member of the discharge planning care management team. This position included: - Conducting needs assessments with patients and connecting them to appropriate resources. - Referring patients to in-patient rehabilitation centers, psychiatric hospitals, home health, long term acute care hospitals, nursing homes, assisted living facilities, and hospice. - Educating patients/caregivers on how to navigate the healthcare system, connected them to resources, and empowered them to utilize those resources. - Collaborating with interdisciplinary healthcare providers and teams.
