- Providing comprehensive pharmacotherapeutic care for all patients at DRHC including medical, critically ill, LTC, and surgical patients- Developed clinical pharmacy services for all inpatients at DRHC, including a comprehensive pharmacotherapy assessment and follow-up program for each patient, a formalized medication reconciliation program at admission and discharge, and VTE prophylaxis quality checklists; created a structured work-loading system for clinical pharmacists’ activities;- Co-Chair for the Regional Antimicrobial Stewardship Steering Committee in Northwestern Ontario- Working as part of the Pharmacy and Therapeutics, and Formulary Committees, providing pharmacy-unique perspectives; introduced multiple medications for addition to formulary; led formulary review process to guide rationale prescribing of medications in hospital and to ensure appropriate inventory of medications for all necessary indications/clinical scenarios at DRHC- Developing quality improvement and quality assurance processes in order to facilitate hospital compliance with regards to OCP reviews and Accreditation Canada’s Medication Use standards; significantly improved organizational adherence to assessor standards
