I conducted Diagnostic Interviews, determined eligibility for services through the Federal State VR Program in accordance with agency laws and regulations. I completed Comprehensive assessments by gathering documentation and data on consumer unique strengths, priorities, abilities, capabilities, resources and interests. I met with consumers, programs and employers to develop well planned VR Services Delivery in an Individualized Plan for Employment (IPE). I coordinated vocational rehabilitation services through the IPE and provided counseling by removing impediments to employment. performed job placement services and networked with employers to develop competitive placement positions. My caseload was comprised of 120-150 adults diagnosed with multiple disabilities. I specialized with those with traumatic brain injury and spinal cord injuries. I monitored community rehabilitation programs and services and facilities. I mentored VR Students placed in internships, mentored and trained staff in the unit. Supervised a Rehabilitation Service Technician.

Conducted vocational rehabilitation counseling and case management with adults with disabilities.. Provided case service coordination duties including: gathering and documenting vocational, biographical, educational data, and assessing medical records to determine case eligibility. Maintained and documented regular consumer contact, delivered timely services, performed and interpreted case assessments. I developed and coordinated comprehensive employment plans that lead to successful employment outcomes. I conducted labor market analysis and job placement services including resume writing, employment interviewing skills training and job lead presentation.

Performed case management and service coordination duties with adolescent consumers diagnosed with developmental disabilities. Coordinated assessments, appropriate interventions and recommendations. Educated and assisted families with applications to pertinent generic services including the Medi-Cal, Social Security, Vocational Rehabilitation programs and services through the public education system. Documented service plans and progress in collaboration with consumers and families punctually.

Provided vocational rehabilitation counseling, case management, job placement preparation services to individuals with developmental, emotional, and physical disabilities. Participated and lead team-oriented meetings with industrial production staff. Participated in Human Rights Committee that reviewed behavioral intervention plans at the facility. Performed diagnostic vocational evaluations that included: vocational interest inventories, career decision making instruments, and vocational ability and aptitude assessments.
