I am responsible for identifying and interacting with medically and psychosocially complex patients and families who are likely to benefit from care management and meet high risk criteria and for coordination of discharge planning services for these patients in collaboration with RN Care Management and other members of the care team.I provide crisis intervention services to all patients deemed appropriate by ED physician using strength based, solution focused approach.I provide evaluation for chemical dependency and psychiatric care, utilizing Interqual and ASAM criteria to guide the recommendation for appropriate level of care.I work with services as a resource in conjunction with Crisis Response Services and other community agencies. Children’s Intensive Outpatient Services supports children and their families in their community in the least restrictive environment using the Wraparound process (WISe). Wraparound honors the family's voice and choice identifies strengths, builds natural supports, and fosters self-sufficiency. Wraparound teams provide comprehensive behavioral health services and support to Medicaid-eligible individuals, up to 21 years of age, with complex behavioral health needs and their families. The goal of the program is for eligible youth to live and thrive in their homes and communities, as well as to avoid or reduce disruptive out-of-home placements. Assessment, treatment, and support services are provided in the youth and family’s natural setting, where needs and challenges present themselves (such as the home, school, and community). Families report excellent outcomes including, youth/family stability, increased support, connection to their community, and hope for the future.I will be the Therapist who works as part of a multi-disciplinary treatment team to provide an individualized, creative, and strength-based approach within the wraparound model. My position will support youth and caregivers to maintain children in the least restrictive and community-based environment.As a therapist, I will deliver clinical services using evidenced-based practice models. This position also participates in an after-hours crisis/emergency response rotation, including initial crisis assessment, safety risk, and outreach if needed.
