Nurse

Provide crisis intervention and case management in domestic violence shelter for adults and children. Responsible for proving support and case management to adults and children with severe mental health disorders in the shelter. Attend to resident needs and provide community referrals. Answer domestic violence hotline and provide crisis counseling, support, and referrals to callers.

Responsible for obtaining HMO authorizations for community mental health and Targeted Case Management. Keep detailed reports of clinician documents, tracking, and assist in coordination of care. Coordinate with staff and community organizations on client care needs, progress, and discharges. Responsible for maintaining quality assurance in client documents to adhere to Medicaid/ Medicare Guidelines. Responsible for decreasing billing errors; while increasing revenue. Provided guidance to a team of over 25 staff members, while keeping a case load of clients.

Responsible for providing advocacy and case management to adults and children with trauma history and mental health disorders. Help expand the advocate office of the Nurse Examiner Program. Provide daily reassurance calls to clients, and provide crisis counseling for clients. Help keep confidential client records organized and up to date. Attend court hearings with clients, attend workshops and trainings and, talk to community organizations. Provided case management to adults and children with DSM diagnosis with finding affordable health care, housing, transportation, mental health counseling, and various community resources. Applied for grants and donations for the Nurse Examiner Program. Provide supervision for volunteers with the program and training for new staff.
