I provided supoort in many settings supporting individuals with various needs from mental health to learning disabilities

While at the Action Group i was a member of a transition team reporting directly to the team manager, supporting individuals on a one to one basis with varying degrees of mental health problems, eating disorders, learning and physical disabilities and helping them to develop Independent daily living skills in accordance with their care plans.
