CurrentPrioritize monitoring and delivery of all aspects of chronic care for up to 6 patients’ within the neuro, med-surg, and trauma units, including all intake and discharge responsibilities, along with delegating care according to applicable skill level or health care needs to other units.▪ Advocate for continuum of care by facilitating patient recovery process and coordinating post care after discharge.▪ Represent department on the skin prevalence committee and establish process for nursing interventions related to various medical, psychosocial, emotional, patient pain tolerance, leading to improved quality of care.▪ Effectively coordinate new and transfer patient care through performing patient assessment and leveraging details to provide individualized education and counseling to diverse population.▪ Identify and treat hemodynamic compromised patients and treat post-surgical patient joint replacement, trauma/fractures, compartment syndrome/ post fasciotomies, stable post op neuro patients (back surgeries, cervical/lumbar fusions, craniotomies).▪ Champion patient pain management and respond accordingly with position changes, cold therapy, activity, timely administration of analgesic with consideration of prior function (chronic pain, assistive device, emotional requirement).▪ Adapt to caring for patients with highly traumatic injuries (e.g., severe falls, gunshot victims, stab wounds, etc.) along with providing post-surgery care for those with scheduled procedures under less
