Geriatric Case Manager
Current2017 Employee of the Year*Working with elderly Holocaust survivor clients and their family members, and other senior citizens , community representatives, agency staff, volunteers, and funding sources with a sensitivity and responsiveness to cultural differences.*Monitoring and coordinating the delivery of evaluations and functional assessments to identify the needs of clients. Developing an individualized, comprehensive service plan for each client.*Assisting clients in accessing and being maintained in needed services as identified in the individualized care plan.*Monitoring the clients overall habilitation/rehabilitation goals.*Determining the need for and arranging provisions of services not provided by the Agency, e.g. financial benefits, legal services, housing, etc.*Assisting clients with applications for financial and service entitlements including those related to emergency grant payments, insurance benefits and other entitlements specific to Holocaust survivors. Referring clients to necessary counseling regarding Holocaust Reparations and additional Claims Conference benefits.*Arranging client transfers to other programs or living arrangements when utilization reviews indicate the need for change.*Identifying problems that prevent clients from accessing or attending needed services and working with providers to develop alternatives for resolving those problems.*Assisting in developing formal and informal support activities on behalf of clients.*Providing direct crisis intervention services for clients as necessary.*Assisting clients in achieving objectives and maximizing independence and productivity through support and training and help in using personal and community resources.*Assisting clients in increasing social support networks of relatives, friends and volunteer organizations that mitigate life stresses resulting from current physical and/or mental conditions.*Satisfy departmental productivity standards
