As case manager I collaborate with the health care professionals involved in the patient’s care to support the patient’s appropriate and effective utilization of health care resources in an effort to gain positive healthy outcomes. The case manager works in collaboration with a multi-disciplinary team approach to educate patients to utilize preventative health care screenings to support overall health and wellness .The case manager provides guidance to patients to utilize homecare and public health services to support their level of health and well being.

Responsible for the management of health care services provided to district's student population. Manage nursing staff of 12 school nurses. Guide and support school nurses in implementing current health policies established by CT Dept. of Public Health, CT Dept. of Education, and local Board of Education. Also, serve as Title IX coordinator for district.

Provide primary nursing care services to medically fragile & chronically ill children--Similar work done with other independent and hospital-affiliated home care agencies in CT, serving both children and adult patients, prior to joining this particular agency.

Provide primary nursing care services to infants/toddlers with medical & respiratory care needs in an inpatient pediatric rehabilitation facility.
