As the Director of Community Integration & Primary Health Care at Halton Healthcare, I lead strategic initiatives to enhance integrated, patient-centered care across hospital and community settings in collaboration with the Connected Care Halton Ontario Health Team (CCHOHT). My role includes overseeing programs like SCOPE, Remote Care Monitoring, and Halton@Home, ensuring seamless transitions between hospital and home. I manage Social Work & Discharge Planning to improve patient flow and prevent readmissions, and lead Bundled Care Programs, Stroke Community Rehab, and specialized WSIB clinics, aligning them with evidence-based practices. I drive integration efforts in Long-Term Care, Supportive Housing, and access programs such as Central Intake and One Link. Additionally, I work to strengthen Halton Healthcare’s primary care network, supporting collaboration with community physicians and the evolving Primary Care Network. With a focus on quality improvement, I champion data-driven approaches to optimize care delivery, safety, and outcomes across all program areas.
