Case Manage and provide wrap around services to youth with disabilities and their families. Partner with High School Case Managers, Counselors, and Behavioral Therapists to advocate and assist in coordinating better services for each youth. Assist families with translation and emotional support during all meetings, events and referral intakes with agencies such as Parents Helping Parents, The Sobrato Center and Silicon Valley Independent Living Center.Collaborate with the Social Security Administration, Department of Rehabilitation, Santa Clara County Office of Education, San Diego State University, Santa Clara Unified School District, Milpitas Unified School District and many other agencies to coordinate services and provide an effective referral process for all participating families. Responsible for data collection and documenting of all interactions along with reporting and attending regional meetings.

Development, Creation and Implementation from ground zero of the NOW Phase I Program. Developed and delivered a 12 week employability skills curriculum to be facilitated under various circumstances and varying number of students withing a correctional facility. Engaged 200+ inmates per week in Job Readiness and Facilitated Moral Reconation Therapy. Designed and Developed a Data Tracking System. Regularly updated, monitored and analysed data.Participated with a team in development of Proposals for Grants as well as Quarterly Reports and attended monthly Providers Meetings.
