• Led change management and end-to-end implementation of Ocean eReferral, an electronic referral system, for providers and organizations across the Toronto region as part of the provincially funded Ontario eServices Program

• Supported multiple initiatives of the North York Toronto Health Partners Ontario Health Team (OHT) through various phases• Coordinated the daily operations of the OHT’s backbone team to support success in the Collective Impact model of collaboration• Planned and facilitated working group meetings, capture minutes and track action items to support the success of for several of OHT groups/structures such as the Patient & Caregiver Health Council, Primary Care Advisory Council and Stewardship Council• Prepared regular communications, presentations and updates for the core members of the OHT • Performed stakeholder engagement to facilitate and enable the completion of OHT deliverables for the Ministry of Health such as the Patient, Family and Caregiver Partnership and Engagement Strategy, Primary Care Communications Protocol, Harmonized Information Management Plan and Quarterly Reporting • Provided support for other OHT priority areas, processes (e.g. Strategic and Operational Planning), short-term projects/programs and other initiatives as required

• Supported the Rural Hastings Sub-Region Integration Table (RHSRIT), a planning table comprised of health and social services organizations, patient representatives and Indigenous advocates, focused on health promotion and prevention, early identification and intervention in primary care and acute care strategies to address Chronic Obstructive Pulmonary Disease (COPD)• Facilitated working groups focused on health promotion and prevention strategies (i.e. improving access to smoking cessation services and patient education) and early identification and intervention in primary care for COPD (i.e. screening appropriate patients and accurate diagnosis with spirometry)• Developed performance indicators based on Health Quality Ontario’s COPD Quality Standard for evaluation of the COPD care pathway used in primary care• Supported the development and implementation of a Community Respiratory Therapy Model of Care for Central Hastings with the goal of improving patients’ self-management and allowing them to stay in their chosen home environment, while being cost efficient to the health care system• Worked collaboratively and built trusting relationships with RHSRIT members and stakeholders • Created communication materials to keep partner organizations and stakeholders informed of the progress and initiatives of the RHSRIT’s working groups• Represented the Rural Hastings Sub-Region at the South East Lung Health Network, a regional community of practice, and conveyed pertinent information back to the RHSRIT Co-Chairs and members• Performed administrative duties for the RHSRIT such as scheduling meetings, setting agendas, taking minutes and preparing documents for distribution
