As the Director of Assisted Living and Memory Support at Edgewater, a Wesely Life Active Community, I help to support the needs and goals of our residents to help them live happy, healthy and active lives.

Working as part of the Home Based Community Services team, I coordinate care for the private paid Personal Services clients to ensure they can remain safely and comfortably in their homes longer. I work to promote services, grow client base and coordinate care across service lines. I supervise, train and educate home care aides and manage client schedules.

As as Physical Therapist Assistant, I provided therapeutic interventions to meet our patients goals to return to prior level of function after surgery, illness or injury and increase independence.

Enrolled over 150 individually insured members in 14 counties in face to face health coaching. Succeeded in helping members reduce risk of chronic/controllable diseases through lifestyle modification. Conducted coaching sessions with clients in person, telephonically and electronically.Maintained meaningful, supportive relationships, encouraging clients to take small, steady steps towards achieving their goals.Provided on site health seminars to the community and partnered with local hospitals, recreation centers, and grocery stores to promote wellness.Implemented health fairs, personal health assessment campaigns and health screenings and conducted follow up appointments with participants.Trained colleagues on health education seminars.Collaborated with various work streams for marketing and referrals to services to ensure success in all areas the Health and Wellness Institute.
