subu, inc. was started in 2005 as a way to indulge my need to create. my primary modality is paper - using it to create books and notepads. i am fascinated with bookmaking and the variations that it allows. the amazing range of materials and the technical detail it requires combines the gifts of my mother's creativity with my father's scientific inklings.about the books: i try to use recycled materials when i can. not because it's trendy or the hip thing to do, but because i feel that it is the right thing to do. i strive to find recycled materials, whether it is the cardstock used to cover the books to the paper inside or buttons that have been saved from the perils of misuse.i am constantly discovering new techniques and re-mastering old ones. continuously challenging myself to learn and grow. i love to collaborate with other people; their energy and enthusiasm help bring new life to my work.

The Account Representative is the key individual who interfaces with Customer Accounts. The Account Representative pro-actively establishes relationships with key organization contacts in each account and coordinates implementation and ongoing account activity during the contract period. Activities include: organizational consultation on EAP policies and practices; development, coordination, and implementation of promotional strategies; oral and written communication with key representatives of the account organization; listening to the customer and assessing the EAP role in supporting organization goals related to productivity and health.

Provided Employee Assistance services, resource and referrals and consultation on various EAP and work/life issues to customers, employees and members of their families who are experiencing personal or work-related problems.

Responsible for the provision of individual, group, and family therapy as well as case management activities consistent with Crestwood’s philosophy. Facilitated individualized treatment planning and provided consultation and assistance regarding implementation strategies with program supervisors and staff as needed. Monitored the clients’ community, including programmatic components of daily living. Provided recommendations regarding the optimization of Crestwood’s policies and procedures in the treatment of clients. Conferred with clinical, residential, educational, vocational, recreational, and health services providers regarding client needs, status, and progress. Conducted pre-admission evaluations as necessary and wrote reports of evaluation. Carried a caseload of 10 to 12 clients at one time. Developed skills in functioning as part of a treatment team to serve the best interests of the young person as well as working with families to maximize a young person’s treatment outcome.

Investigated allegations of sexual, physical and emotional abuse against children and young people under the age of 16 years. Also investigated allegations of neglect. Completed Initial and Core Assessments with the young people and their families in the home setting. Carried a caseload of 12 to 15 families at one time. Worked on the front line, receiving referrals and making all necessary checks with the police, school and general practitioners on all referrals made to Social Services. Developed skills in working with culturally diverse populations and liaising with outside agencies.

Provided individual and group therapy to behaviorally and emotionally disturbed adolescents attending the Epworth School. Wrote master treatment plans, quarterly progress reports, functional behavior assessments, and case documentation for each client. Developed and implemented a group module used with adolescent females attending the school.

Answered hotline calls, compiled and updated community resource manual, assessed individuals for admission into shelter, performed intakes, provided initial assessment of residents for the therapists, provided group therapy, observed individual and group therapy sessions, developed service plans and discharge summaries, attended various training sessions, participated in network meetings with other area agencies, provided structured activities for youth residents, and covered shifts for resident counselors as needed.

The Fishbowl provides therapeutic group services free of charge for parents and siblings of children and adolescents with mental illness as well as adolescents who are dealing with mental illness. Co-facilitated group for siblings of children with mental illness aged 7 to 13. Also co-facilitated a group for children affected by mental illness.
