Clinical Social Worker
Current• Social worker for up to eight patients• Participate in daily team meetings with psychiatrist, case worker, and nurse to discuss how patient is doing on the unit, any medication changes, and when the patient is slated to be discharged • Meet with patients everyday to discuss discharge planning, outpatient resources, safety planning, etc. • Make collateral phone calls, scheduled outpatient appointments, and make referrals to outpatient resources• Facilitate group therapy sessions to discuss coping skills, anxieties about discharge, mindfulness skills, etc. • Certifications: Pre-admission Screening and Resident Review, December 2023
