Leading and effectively implementing and monitoring population health, health development and integrated models of healthcare delivery across the whole of the organisation and actively develops strategic partnerships with key stakeholders.• Providing operational and strategic leadership in the development of the organisation wide consumer centred integrated care strategies and care for vulnerable population groups.• Providing strategic advice to the Chief Executive and the Executive team regarding the development and implementation of out of hospital health care delivered in the community or ambulatory setting.• Working collaboratively with acute care teams in hospital avoidance strategies and to pull appropriate activity into ambulatory or home settings.• Developing and sustaining strong partnerships with other health providers, local government, Federal and State agencies, local communities.• High level negotiations and strategic liaison with other service delivery agencies and development of collaborative co-commissioning models and integrated service networks to deliver cost effective services.• Management of key performance indicators and outcomes of priority programs and ensuring these services are responsive to changing community needs and government priorities• Development, implementation and coordination of integrated health initiatives, population health and community health services within MLHD to improve health outcomes and patient experience of care, better co-ordination of services between primary, community and hospital settings and working towards a more efficient health care delivery system.• Budget management, systems and process development, contribution to strategic planning and continually improving services to ensure they are safe and high quality, responsive, cost-effective and meets the needs of the community.
