The Zorya Foundation is a non-profit dedicated to creating opportunities for women in healthcare. We support various initiatives, including childcare grant programs, scholarships and education support efforts and advocacy to advance opportunities for women in the healthcare sector. We exist to fill the gaps so that women can lead in healthcare, across all professions.

Support the clinic’s mission of "providing high quality sexual health services to all in an affirming environment" by contributing to strategic planning initiatives, developing clinic policies and procedures, implementing quality measures for HIV testing & counseling, recruiting and retaining volunteers, and bi-weekly volunteering within the clinic. Serve as the board’s secretary keeping detailed records of meetings and tracking strategic planning initiatives.

Provide intensive case management services, care coordination and individually tailored patient navigation to priority HIV+ populations with goal of successful linkage and retention in HIV care. Focus on identifying and addressing social determinants of health that impact access to care and social services. Forge strong relationships with patients by providing culturally-sensitive, trauma-informed support to reduce HIV-related stigma and increase comfort with and motivation to engage in medical care. Focus clinical and community-based patient interactions on education by promoting health literacy, providing HIV disease education and medication adherence counseling, and providing non-judgmental sexual health and HIV/STI prevention counseling. Serve as the primary representative of multidisciplinary teams, collaborating extensively with clinicians, community clinics and organizations, and other care and service providers to maximize adherence to and retention in care. Develop collaborative relationships with local providers, partner agencies and community-based organizations, community hospitals and clinics, local health departments and testing sites, and other care and social service providers to promote services and accept referrals for the program. Serve as our team's "Housing Program Specialist." Lead consultant for colleagues and peers assisting HIV+ patients who are impacted by homelessness and housing instability. Interface with community-based organizations and clinics to advocate for access to housing programs for patients living with HIV.Maintain high quality compliance with program standards and protocols, which include conducting comprehensive assessments and harm reduction screenings, developing individualized service plans, medical records documentation and data reporting to funders.

Worked as a "HIV Medical Case Manager" in a clinical setting to support and educate patients living with HIV; performed crisis intervention; offered support and referrals to respond to the immediate patient needs; educated and counseled on health literacy and harm reduction. For two years, served as the sole Medical Case Manager at ARCW-Kenosha, running all social service programs, and assisting with HIV, STI, and needle exchange prevention programming, from this site. Recipient of “Leadership Award” in 2014. Worked as a primary clinical care team member to advocate for patients, and facilitated communication with clinical staff, clinical support staff, external disease management, and other care and service providers as appropriate. Complied with state and agency standards by conducting assessments, developing individualized service plans, medical records documentation, and maintained compliance with policies to assure patient confidentiality.
