Los Angeles

Provide case management services to youth enrolled in the Youth Squad 360 program. The demographic serviced consist of youth ages 17-24 who are currently working or have worked for Summer Night Lights as a Youth Squad member. The Youth Squad 360 program utilizes the Maslow's Hierarchy of Needs model to assess the immediate needs of the youth enrolled. As a case manager my role is to provide mentorship, be an additional support system, and help link the youth to resources needed to achieve success in their academic, professional and/or personal lives.

• Interviews participants receiving public assistance to obtain financial and other eligibility data. • Assists participants or new applicants with completion of required documents to apply for benefits, continue receiving assistance or make any necessary case changes.• Reviews and analyzes financial and eligibility information to determine initial or continuing eligibility for one or more public assistance programs (CalWORKs, General Relief, CalFresh, or Medi-Cal)• Contacts individuals, agencies, insurance companies, or other institutions to verify information provided to resolve discrepancies.• Electronically enters information of benefits for eligible applicants or participants of public assistance programs.• Refers applicants or program participants to appropriate County department and other agencies for services and resources to address their immediate and long-term needs. • Searches various income and eligibility related computer databases to confirm if applicants or participants of the programs are receiving income from other sources of public assistance aids or programs, and verify eligibility status.• Provides participants with job referral and placement programs to applicants and participants recipients of public assistance.• Verifies earnings or incomes to compute the amount of over-payment or over-issuance, generate over-payment reports and Notices of Action, and, when appropriate, makes referrals to the Welfare Fraud Prevention and Investigations unit.

I Provide Case Management Services for youth who participate in the GRYD program. My duties include, but are not limited to, Advocating for the youth and their families, provide the youth and his family with the necessary tools needed to succeed in the GRYD program such as; anger management, individual and family counseling, drug classes, etc., offer information concerning legal matters and help locate services that can enhance the youth's grades and overall academic progress.

Ensured the safety and well being of youth who have been placed at Hillsides home for children. I assisted in helping these youth develop healthy relationships and healthy habits. I also ensured that their day to day needs were provided to them. Finally, I provided stability and consistency to their day to day lives.

My primary responsibility at North East Juvenile Justice Center (NEJJC) was to oversee and manage the Teen Court program, A Diversion program that functions as an actual court for young people who commit non-serious crimes, in which they are questioned, judged and sentenced by a jury of their peers. Teen Court provides an alternative to Juvenile Court, helping the youth avoid harm caused by unnecessary involvement in the juvenile justice system.Responsibilities; I. Screen Police reports for appropriate cases for the Teen Court Program II. Meet with Families and Youth to explain the program in detail and enroll youth. III. Collect necessary Data/Documentation from youth and families and Schedule the Youth for Teen Court IV. Complete summary of events, and all other necessary documentation for Teen Court and Forward completed packets of documentation along with original police report to the Judge V. Attended Teen Court proceedings as official Liaison to the Los Angeles County Probation Dept. and Meet with youth after the Teen Court Proceedings to debrief and provide copy of Teen Court Conditions of probation. VI. Maintained a caseload of over 60, WIC 654-informal probation youth, Providing case management services to help the youth comply with their conditions of probation.Other Responsibilities;I. Acted as Preceptor to B.S.W. and M.S.W. internsII. Provided assistance to the Clerical Unit or Acted as Receptionist.III.
