Senior Care Behavioral Health UnitResponsible for providing individualized, age-specific, goal directed care. Management of most acute and chronic medical needs in conjunction with emotional or behavioral treatments. Perform physical and behavioral health assessments in order to follow a plan of care including administering prescribed medication and then implementing and evaluating the plan based on the patient response to treatment. Documentation of patient behavior, status, and progress to the administered plan. Provide education to patients and family members regarding prescription medications and appropriate care.
