My biggest goal is for my mentees to become the best person they can be at their role.I have a maximum team of six people I train to help support our IDD individuals in the community. I have my own caseload of 20 IDD individuals I care for in the community ensuring their health and safety. My mentee team carries their own caseload, in which I train my team how to connect our IDD individuals to their services. I have my team shadow me in the community so they understand how to ensure our IDD individuals live a good quality of life.I will teach mentees how to submit documents and to meet time lines. I will teach mentees how to stay connected with the agency provider to ensure that services are being provided for their individuals they support.This job requires time management and organizational skills, in which I excel. I am cross trained in Home and community based services, Enhanced community coordination, and Pre admission screening and review.

Helping train new service coordinators in order to care for our individuals and connect them with their services. I was responsible for six mentees on my team at a time, in which they received a variety of training. I would take mentees on visits in the community to help them learn how to support people on their caseload. I would support my mentees by seeing a few people on their caseload. My biggest goal is for my mentees to become the best person they can be at their role.

I worked with a caseload of 50 people that were under my care. I would do home visits and see my people I support out in the community. I would connect with their HCS agency provider to ensure their services were being provided. I was an advocate for my people. I would ensure that my annual and quarterly meetings were completed with my people I supported. I displayed time management skills and completed various tasks outside my job description. I would update my outlook calendar on a weekly basis to stay organized.
