• Co-designed, implemented, managed, then expanded the successful CMS demonstration Project for the management of high risk Medicare beneficiaries (2005-2012). Was the only demonstration site that demonstrated both positive clinical and financial outcomes. • 2009 – present, developed and then implemented model of Centralized Nursing Model for post discharge follow-up communication to assure smooth transitions in care• Negotiated a waiver with CMS for the ’72-hour waiver’ regulation; then implemented a management program for the safe transition of patients directly to skilled nursing facilities, when appropriate, to prevent unnecessary hospital admissions.
