Assistant Director Of Adult Case Management
Current• Oversees assigned case managers at Woods. • In coordination with the Director, is responsible for training and development of training materials for Case Managers.• Provides Case Managers with a complete understanding of applicable states funding and social service streams.• Assists in the development of criteria and monitoring program evaluation systems, which address staff training and supervision, health/safety requirements and the tracking and training of all documentation required for the individuals on caseloads.• Responsible for the assurance that all paperwork is submitted according to deadlines and meets the internal and external requirements. • Efficient in the use of a computer for professional correspondence and reports, including creating and tracking data and reports using electronic health record systems.• Provides Case Managers with an understanding of individual benefits available to the clients and where applicable, assists case managers & families with procurement of such benefits, e.g., SSI, Medicaid. • Ensures and assists, as necessary, with the pursuance and confirmation of funding approvals for aging out clients, transfers and clients with special staffing needs in conjunction with case managers.• Provides oversight and technical assistance related to discharge planning, including sharing information and referrals to community resources, when necessary.
