My role as an HCI with the Bridges to Health Program is to coordinate waiver services through the creation and implementation of the individualized health plan.Responsibilities include: Facilitating team meetings assessing the need for services, developing and updating the IHP, linking the child and family/caregiver to the identified services. Consult with the child and caregivers on the progress of goals. Consult and coordinate with local DSS providers, waiver service providers, schools, and therapeutic services.
