With over 15 years of experience in nursing and healthcare management, I am the Founder and Chief Clinical Officer at SparroWell, a leading Chronic Care Management & Population Health company serving senior population Nationwide.SparroWell partners with geriatric provider groups managing the care of seniors, LTC/ALF Operators, and nationwide ACOs to provide personalized and holistic wellness solutions. We have a proven model with stellar outcomes resulting in improved collaboration, increased census, and decreased hospitalizations.As a specialized geriatric nurse, I have the expertise and skills to oversee the clinical operations, quality, and compliance of SparroWell's products and services. I am passionate about improving the health and well-being of our seniors and partners, while working closely with our experienced nurses and partners to deliver innovative and evidence-based solutions that meet their needs and expectations. I also leverage my many years of experience with Optum and my knowledge after being the Director of Nursing in the long term care sector. I am driven by SparroWell’s mission to work right alongside our partners to proactively improve the lives of our seniors, while supporting providers and LTC communities.
