- Assist my clients through the application process to sign up for Health Insurance through the ACA (Affordable Care Act) by navigating the NYSOH's Marketplace website.- Identify and educate my clients on all aspects of the plan they are eligible for through the NY State of Health Marketplace by explaining the benefits covered, monthly Premiums, APTC (Advanced Premium Tax Credit), Deductibles, MOOP (Maximum Out Of Pocket), Co-pays, Coinsurance, and Provider network. - Responsible for identifying and assisting current members who are due to recertify/renew their healthcare coverage for Medicaid, EP (Essential Plan), QHP (Qualified Health Plan), and CHP (Child Health Plus) by completing member’s annual recertification application including adding on additional eligible family members. - Conduct home visits and other appointments as needed to complete the application and obtain all required documentation. - Establish and maintain good working relationships with external business partners such as hospital and provider organizations, City agencies, small businesses and Community Based Organization personnel where enrollment activities are conducted. - Participate in health related activities / special events, such as local or out of town health fairs or conferences.- Refer clients to internal services if it is suitable for them; such as Care Management, SEP (Syringe Exchange Program), On-site clinic for STD/HIV Testing and or treatment.
