Responsible for the care and supervision of a group of adolescent clients (21 & under) in the Residential and/or Day Program areas. Responsible for establishing a positive, supportive relationship with clients. Responsible for providing a warm, homelike atmosphere throughout the residential unit and assists clients in maintaining their personal space/possessions.Responsible for notifying immediate supervisor of any deficiencies in his/her assigned area and/or unit, which may be in violation of regulations/standards, which govern the facility. Responsible for following Policies and Procedures of Woods Services.Responsible for maintaining an accurate inventory of personal clothing and other personal possessions of assigned clients. Responsible for maintaining a positive working relationship with all supervisory, support, peer and other Woods employees throughout the Residential Unit and Day Program areas.

• Assessing the needs and outcomes of each client and developing an ISP (Individual Service Plan) with the assistance of the individual and those apart of the care team.• Coordinating the consumer and family with service providers and generic community resources also identifying services as requested by the individual and their family.• Monitoring services to assure that the ISP is implemented, that the ISP continues to meet the needs and desires of the individual and that the health and safety of the individual is assured.• Maintaining eligibility information and assisting individuals to apply for assistance for which they are eligible and advocating for needed services not readily available to the individual.

• Work with children with mental illness or behavioral disorders need support and supervision to help them meet treatment and educational goals.• Helps manage the behavior of children and adolescents who have problems such as autism, in a community setting.• Analyze the information they collect or receive about each client, and use problem-solving skills to find treatments or solutions. • Patience is another important quality, as is the ability to communicate with clients, families and other staff.
