*Implement and manage Drug Management Program (DMP) for medicare clients to ensure the program is compliant with the Center for Medicare and Medicaid Services (CMS) guidance.*Collaborate with Medication Therapy Management (MTM) vendor to develop business and clinical strategies to optimize performance.*Review Prior Authorization Criteria Updates quarterly. *Edit/Update Drug Monographs for preparation of Pharmacy & Therapeutic (P&T) meetings quarterly. *Present new-to-market drugs to P&T Committee in the quarterly meeting. *Review/Analyze the weekly Medicare Medispan Report and provide benefits team instructions to update the UM (utilization management) edits, add new medications or line extension to formulary. *In charge of the 2024 formulary, monthly formulary and monthly UM changes submission to CMS. *Partner with Clinical Account Management to evaluate client feedback for potential enhancements. *Work with internal operational teams to execute innovative clinical programs and continuously improve the process. *Provide clinical consultative support and subject matter expertise across the different teams at the organization.*Review physician inquiries for pharmacy prior authorization requests based on established pharmacy guidelines and contract criteria.

*Active involvement in interdisciplinary patient care activities, medication therapy monitoring and providing drug information, education to both healthcare providers and patients.*Manages high volume (+500 patients) of patients with Autoimmune Disorders, Immunodeficiency, Neuromuscular Disease, Crohn’s Disease, Ulcerative Colitis, Multiple Sclerosis, Alpha 1 Antitrypsin Deficiency and other Rare Diseases daily to ensure patients receive infusions on the schedule. *Communicate directly with patients or their families on all aspects related to medication therapy to promote medication education and adherence. *Admit new patients into service.*Facilitate insurance approval process.*Perform drug utilization review (DUR).

*Clinically review, verify medication orders and following department protocols to ensure safety and appropriateness for medication dispensing and administration.*Check technicians prepared medications or non-sterile compoundings before being delivered to pyxis and medical floors.*Provide medication information to physicians and nurses.*Document clinical interventions.

*Rounding with a multidisciplinary team on medical floors and ICU.*Review and verify medication orders entered through CPOE system and provide drug information to various members of the healthcare team.*Work closely with nurses to ensure proper administration of medications.*Oversees the intravenous admixture area once a week to ensure all IVs products follow the USP 797 guidelines and are prepared correctly.*Supervises central pharmacy technicians once a week to maintain on time medication deliveries to all patient care areas throughout the hospital.*Use MediTech, CPOE, Pyxis system daily.*Anticoagulation Experience Manage warfarin dosing for all patients: order labs and adjust dose for approximately 25 patients with thrombotic disorders daily.*Antibiotic Stewardship Experience
