As an ACNA Wellness Assessor I am part of the Regional Assessment Service in delivering CHSP assessments on behalf of My Aged Care.The role is diverse and a key element is being able to conduct a conversational assessment whilst completing necessary documentation to be able to develop effective support plans which reflect individual needs of consumers aged 65+.I also refer and liaise with a wide group of Service Providers to ensure the best possible outcome for my clients.

Working in collaboration with multiple organisations (state government, federal government and non for profit agencies) to deliver peer education programs for seniors on a range of health and wellbeing topics. This involves compiling training manuals, facilitating training and running workshops, as well as report writing, researching and extracting data and managing budgets. It also involves the recruitment of peer educators and training of peer educators to deliver a range of programs to our local community, within metropolitan Adelaide and also targeted regional centres.

City Of Campbelltown, South Australia

Managing the Healthy Communities Initiative funded by the Australian Government (budget of $546,000) to support and increase the local communities knowledge about nutrition, being physically active and staying socially connected through education, events and exercise. With the support of a steering committee we developed a unique branding and I was involved in the development of the website www.easternhope.com.au which continues to be sustained by council. Networking and developing partnerships was critical to this role. I also delivered 6 monthly reports which required the collation of significant evaluation data.
