• Organize community entry, mobilization, and sensitization• Supervise, train, and guide community health workers• Establish a feedback process and prepare scheduled reports for the Organization• Ensure outreach and follow up of clients discharged from the hospice• Make complex home visits to assist and mentor CHWs• Provide emotional support to family, patients, and staff.• Follow up on children and family of deceased patients• Oversee and supervise support outreach programs• Report back on support outreach programs• Manage patient cases from beginning to end. Case management involves assisting with admission, transitioning care, and discharge from the hospice program.• Offer counseling services to patients and their families.
