Provide direct patient care for veterans with spinal cord injury in the outpatient setting. Collaborate with all team members to include Social Work, dietary, psychiatry, pharmacy, physical therapy, and primary care providers. Coordinate with other facilities and agencies as needed to ensure continuity of care for patient recently discharged form outside hospitals or under home health care or hospice. Make daily inpatient rounds with the SCI team and provide input to the plan of care for spinal cord injury patients while hospitlaized. Serve as the resource nurse for the hospital staff on the special nursing issues related to spinal cord injury patients.
