About Frank Verbeek

We moeten naar een nieuw gezondheidssysteem. Waarin er een focus is op brede positieve gezondheid zoals veerkracht, zingeving, sociaal blijven, mee kunnen doen in de maatschappij, omgaan met uitdagingen van het leven. Hierbij is het belangrijk de informele (0e lijns) zorg te versterken, dicht in de lokale gemeenschappen, en de regie bij de oudere te leggen. Door niet alleen mensen die ziek zijn of een hoog valrisico hebben te helpen via de gangbare secundaire zorgprogramma’s, maar naar iedere oudere te kijken, wordt preventie primair: voorkómen dat iemand ziek, valt of eenzaam wordt. Er is, complementair aan de huidige ketenaanpakken, een gemeenschapsgerichte aanpak nodig vanuit het sociale domein. Van daaruit wordt verbinding met het formele gemeentelijke en het medische domein gelegd. Deze systeemtransitie maakt het brede gezondheidsprogramma “Maak Rimpels” mogelijk. We bieden een sociale innovatie aan waarmee oudere volwassenen nieuwe netwerken vormen en gezondheidsvaardigheden ontwikkelen. Groepen van ongeveer 15 oudere volwassenen doen onder leiding van verschillende coaches mee aan vier onderdelen: positieve gezondheid, digivaardigheid, vitaliteit en samenredzaamheid. De deelnemers worden ondersteund door materialen zoals handboeken, maar ook met technologische innovaties zoals het “Mijn Maak Rimpels” platform met het digitale beweegprogramma volledig gebaseerd op Otago, de GoLiveClip wearable voor activiteiten monitoring, inzicht krijgen in je valrisico, je loop asymmetrie en je loopsnelheid en sociale alarmering. Ten slotte maken we dit mogelijk met een systeeminnovatie waarin we nieuwe structuren opbouwen voor regionale samenwerking tussen het sociale domein en zorgdomein. Maak Rimpels versnelt de urgente gezondheidstransitie met brede positieve gezondheid en informele zorg in een gemeenschapsgerichte aanpak, waarbij de oudere volwassene zelf aan het roer staat. Dit leidt tot drie vormen van preventie:- Valpreventie, door valangst en valincidenten te verminderen en daarmee de SEH contacten;- Eenzaamheidspreventie wordt bereikt door het vormen van hechte sociale netwerken;- Zorgpreventie door uitstel of vervanging van formele zorg verlaagt de druk op professionele en mantelzorg. Hieruit volgen onderaan de streep lagere zorgkosten voor de maatschappij.Gociety was established 12 years ago and focuses on social innovations in the field of prevention for elderly adults, by means of "artificial intelligence" (AI) and “assistive technologies” (AT).
