Community Mental Health Practitioner in the Personal Helpers and Mentors program (PHAMS), providing support to people with mental illnesses in overcoming social isolation and increasing connections in the community. I provided one on one client contact to people out in the community creating and working from individual recovery plans and using recovery based practices in order to support my clients in reaching their goals. My work at Mind Melbourne Inner South included co facilitating a woman’s group where I designed a twelve week program including workshops aimed at broadening skills, building strengths, networking and increasing self esteem and tolerance and understanding of others. My work involved daily meetings and reporting on clients, up to date case noting, rostering, minute taking, handovers and leasing with many other health professionals (including but not limited to) psychiatrists, psychologists, general practitioners and social workers. I worked with external orgaisations such as housing and accommodation services both government and non government, Mission Australia, job networking orginisations, Centerlink and a variety of neighborhood centers and community centers. I worked from the office and out in the community as well as in people’s homes to best assist my clients to achieve their goals.

I worked as a mental health case worker with clients suffering mental illness. My clients were from marginalized backgrounds living in community housing. My task was to establish these clients within their communities as well as to minimize their risk-taking behavior and hospitalizations. Part of my role was to act as a mediator between my client and government organizations working through a goal orientated program off personal support plans which we built while working together.
