❖ Conduct intake services with clients to perform a needs assessment for housing services and assist clients in developing self-sufficiency plans and timelines utilizing SMART goals.❖ Provide individualized support through direct services, referrals and advocacy for other support services such as Social Security Benefits, SNAP and VA Benefits.❖ Research appropriate housing options for client based on their need, to ensure client meets compliance standards for housing subsidies and housing units❖ Facilitate client trainings on personal finance, budgeting and best decision-making skills.❖ Track and document clients’ progress and outcomes in HMIS system.❖ Collaborate with community representatives, referral sources, leaders of the shelter and serve as liaison between client and community representatives for support services such as employment, transportation and any other needed resources.

❖ Monitored shelter to ensure safety protocols were adhered to and any incidents were properly reported and investigated, including controlled substance counts and medication administration.❖ Conducted security checks to ensure safety of clients and provided detailed notation necessary for the nextshift to be aware of daily activity that transpired during the past shift and detailed each client’s interaction in accordance with program requirements.❖ Input client information in database and assisted with tracking and reporting in HMIS and excel.
