Hi! My name is Gabrielle (she/her), and I am a Licensed Clinical Social Worker in the Washington DC metropolitan area who is passionate about health equity, mental health, and wellness - whether that is in providing therapy in private practice, designing and implementing programs and structures to shift organizational culture, facilitating conversations and workshops, resource development for marginalized peoples, or advocacy. While my experience includes healthcare and community organization settings, most have known and connected with me on my work with Black immigrants and the national racial justice and immigrant rights organization, the UndocuBlack Network. I am now the principal consultant and psychotherapist at Diasporic Healing LLC, a mental health private practice and consulting firm that focuses on clinical and mezzo-level interventions to support and integrate wellness in our everyday lives.     The Mental Wellness Director leads the charge on the Network's Mental Wellness Initiative, which is dedicated to expanding the dialogue, raising awareness, and educating about mental wellness in our communities and designing, creating and facilitating access to adequate mental health and healing resources. As Mental Wellness Director, Gabrielle authored two publications,: The UndocuBlack Guide for Mental Wellness Specialists (a cultural competence support for those in the mental health field) and The Alive and Well Discussion Series (a curriculum to support community members in facilitating conversations about mental health, well-being, and the world around us).     The care management team, which has an ultimate goal of improving health outcomes and enhancing collaboration, is an extension of patient care in between office visits with the Primary Care Provider, coordinating with providers to make sure the patients are getting the care and services they need.
