Diasporic Healing LLC is a mental health private practice and consulting firm that focuses on clinical and mezzo-level interventions to support and integrate wellness into our everyday lives. We are committed to increasing access, equity, and understanding of mental health and well-being among members of the community at all levels through the provision of individual therapy services, clinical and community education and training, and one-to-one and organization consultation.

Gabrielle has stepped into the role of Co-Director to support the organization in its growth and continued investment and commitment to Black undocumented immigrant communities.

The Mental Wellness Director leads the charge on the Network's Mental Wellness Initiative, which is dedicated to expanding the dialogue, raising awareness, and educating about mental wellness in our communities and designing, creating and facilitating access to adequate mental health and healing resources. As Mental Wellness Director, Gabrielle authored two publications,: The UndocuBlack Guide for Mental Wellness Specialists (a cultural competence support for those in the mental health field) and The Alive and Well Discussion Series (a curriculum to support community members in facilitating conversations about mental health, well-being, and the world around us). She offers trainings related to both publications.

Gabrielle is one of the co-founders and co-organizers of the inaugural Undocumented and Black Convening, a first of its kind space for Black undocumented immigrants to heal, organize, and build community. As a result of this convening, the UndocuBlack Network, a fiscally sponsored project of a 501(c)3 organization, was created. The mission? The UndocuBlack Network is a multi-generational network of currently and formerly undocumented Black people that fosters community, facilitates access to resources, and contributes to transforming the realities of our people, so we are thriving and living our fullest lives.

The Social Work & Behavioral Health Consultant role on the HQ Care Management Team was created in September 2016 to play a central role in supporting care management for high-risk/complex patients in the markets of the company’s Accountable Care Organizations (ACOs) across the United States. The role is divided into two pilots: Social Work Care Management and Behavioral Health Care Management. Social Work Care Management - Collaboration and consultation with practice-based care managers and patients in West Virginia to evaluate and address complex needs and psycho-social barriers to care. - Social Work Consultations include: conducting comprehensive assessments, patient care and community resource coordination on a multidisciplinary team; coaching and educating care managers; researching and evaluating community resource availability and gaps across markets; and, engaging in ongoing development of the care management technology platforms. Behavioral Health Care Management - Partner with the Mental Health Director to design, develop, implement, and evaluate a new Behavioral Health Care Management initiative, to include piloting different models across markets and serving as the Behavioral Health Care Manager for the centralized model. - Collaboration and consultation with primary care practices and patients in rural Delaware to address behavioral health needs and complexities. - Behavioral Health Consultations and Care Management include but not limited to: conducting comprehensive assessments and screenings, care planning, psycho-education and short-term counseling, community resource coordination, and facilitating ongoing communication between patient and the primary care team; coaching and educating the primary care providers on behavioral health care, needs, and resources; researching and evaluating community resource availability and gaps across markets; and, engaging in ongoing development of the care management technology platforms.

The care management team, which has an ultimate goal of improving health outcomes and enhancing collaboration, is an extension of patient care in between office visits with the Primary Care Provider, coordinating with providers to make sure the patients are getting the care and services they need. The team, which includes a pharmacist, social workers, health advisors, and nurse care managers, aims to support patients who have been identified as needing a higher level of support, given their complex medical issues and high utilization costs. As one of the social workers on the team, Gabrielle helps improve outcomes by conducting psycho-social assessments and determining psycho-social needs, providing short-term counseling and clinical support, and identifying community resources for these high-risk and complex care patients. With her background in behavioral health, Gabrielle provides particular support not only to the patients, but to the staff as well, in connecting the impact of mental and emotional wellness to physical health and conditions.
