As a Care Navigator MSW at CenterWell Senior Primary Care, I've honed an efficient and effective approach to patient care, reducing hospital readmissions through continuous follow-up and personalized care plans. My effective collaboration with healthcare teams and meticulous case documentation reflects a dedicated mission to optimize patient outcomes and uphold the highest standards of privacy and ethical practice. Additionally, I assist to guide new employee onboarding and direct training and have created and maintained a regional community resource and training guidebook. I conduct independent research of market and community resources for support with common social health determinants, and operate as social welfare team lead for the KC region.
