Urgent community response is the collective name for services that improve the quality and capacity of care for people through delivery of urgent, crisis response care within two-hours and/or reablement care responses within two-days.The Humber UCR service is delivered across Whitby, Scarborough, and Ryedale Community, encompassing travel across market towns, the moors, the seaside and the coast within daily working. As a UCR Clinical Coordinator, working in the Community Team, provide clinical triage and care delivery in patients’ own homes and Care Homes.The service operates 7 days a week, for a minimum period of 08:00-20:00, incorporating a multi-disciplinary team (MDT) with a wide clinical skill set, to ensure equitable care provision across the full geographical patch. UCR Co-ordinaries use a robust clinical triage process, accepting appropriate referrals from across multiple partner organisations and through agreed clinical diversionary pathways. The NHS Operational Planning and Contracting Guidance 2021/22 sets out the first stage for implementing the community two-hour crisis response standard in England by March 2022. All integrated care systems (ICSs) in England must, in this year, ensure crisis response care is available to all people within their homes or usual place of residence, including care homes, within two hours. The standard should improve patient outcomes by meeting their urgent care needs at home or usual place of residence in a timely way. The ambition is to provide crisis care within the community to prevent avoidable hospital admissions and accelerate the treatment of people’s urgent care needs.

In addition we ran MSH @ Queens Park, which was a free and confidential sexual health service based within Queens Park Health Centre, London W10. MSH Wellbeing, the trading arm of MSH Health & Wellbeing CIC, provides innovative therapies for the mind and body from the heart of Angel. Specialising in health and wellbeing, our in-house team includes a full-time massage therapist (Renato), male grooming specialist/beautician (Marcelo) and me as the supervising Nurse Consultant/Masseur. We offer a wide range of wellbeing therapies including massage, male grooming, waxing, and intimate body hair removal solutions including Intense Pulse Light (IPL). Working in partnership with a number of Associates, including four psychotherapists, providing couple and psychosexual therapy, as well as hypnotherapy and ear acupuncture. This position started as a secondment from my Nurse Consultant role, as Project Manager for the implementation of Agenda for Change (AfC) across the Trust. I lead a combined team of 30 plus managers and staff side representatives to evaluate and assimilate over 3,500 staff onto the new terms and conditions and pay scales of the AfC system.As AfC was completed, I was appointed as deputy for the Director of Human Resources and Organisational Development, I lead for St Mary's Hospital on the proposed merger with Hammersmith Hospitals' NHS Trust and closer integration for the Faculty of Medicine at Imperial College, to creation the UK's first Academic Health Science Centre. Co-ordinating the St Mary's elements of the HR workstream, I work very closely with Annabel Scarfe from Change FX, on the change management process and staff engagement.

As one of the UK's first Nurse Consultants, this was new role created in response to criticism of the lack of health advisors within the Jeffery's Wing. ♣ Created the Sexual Health Information and Protection (SHIP) team within the Jefferiss Wing, one the UKs busiest sexual health departments, seeing over 200 patients per day.♣ Managed the team of Health Practitioners, Results Administrators and specialist sex work projects (Working Men Project and Praed Street Project).♣ Co-ordinated monthly health promotion campaigns within the department and across the local community.

The Working Men Project (WMP) is a confidential, specialist HIV prevention and sexual health promotion service for men working in the commercial male sex industry in the United Kingdom. Upon my appointment to the post I developed the service from a part-time research interest of a physician, to the UKs leading and award winning service for the provision of sexual health services to male sex workers, recently cited in the Home Office Stategy for Prostitution as an example of best practice.

Initially I started at the Parkside Well Men's Service as a nurse, but soon developed the role into that of a Primary Healthcare Practitioner and was appointed to the role of lead practitioner/manager. The clinic operated on a part-time basis, providing lifestyle advice and basic cardiovascular screening to men within a community healthcare setting. However, due to under investment, lack of appropriate resources and managerial support, and the very part-time nature of the clinic, it was closed in 2001 when its funding was withdrawn.

This was my first clinical position within the field of HIV and sexual health care. I worked initially on Cheere Ward, a 20 bed medical admissions ward on the main King's College Hospital site, with 12 ring fenced HIV admission beds. After a few months, I soon negotaited a rotational placement into the GU out-patients (Caldecot) clinic, where I gained clinical experience in the management of sexually transmitted infections.

My first position upon qualification as a registered nurse, staffing on a 28 bed vascular surgical ward, with 6 bed high-dependancy bay. This was a clinically challenging unit which was a tertiary regional referral centre for acute vascular surgical emergency cases.
