Care Bridge Specialists offer a partnership model that provides vulnerable people with autism and learning disabilities with wraparound teams on a 'bridge to better.'We dedicate our specialist services to working together with Clinical Commissioning Groups, Local Authorities and Private Care Providers to achieve the change in social care and support that we know is not only possible but vital. Our Practitioners are highly skilled experts with an average of 9 years experience working with adults and young people with autism and learning disabilities. They are further supported by an in house team of PBS Lead Practitioners and qualified Counsellors.
