• Assess medically fragile pediatric patients, collaborate with therapists, health care providers, and caregivers in care plan, perform appropriate patient-specific nursing interventions, evaluate effectiveness of interventions• Provide direct nursing care to patients across the life span in accordance with established policies, procedures and protocols• Administer medications and treatments per doctor's orders and monitor for desired/adverse effects of medications, treatments, and procedures• Monitor vital signs, I&O, weights and report change in status to MD• Provide accurate and thorough documentation• Tracheostomy, enteral access (NGT, GT, NDT, GJT), wound care, PICC dressing change, parenteral access (PICC, medi-port, PIV), oxygen therapy, nebulizer treatment, CPT, and nasal, oral, tracheal suctioning experience• Perform ADLs, ROM, PT, OT, ST exercises as ordered• Apply orthotics and monitor for skin breakdown • Maintain patient safety and implement seizure, aspiration, fall, universal and safety precautions at all times• Educate patients and caregivers on proper health practices• Complete patient intakes, reassessments, LVN supervisory visits, and company in-services
