We conducted a retrospective study on 107 patients with hepatocellular carcinoma (HCC), comparing those with metabolic-associated steatotic liver disease (MASLD) to patients with other etiologies at the University Hospital of Modena. The study aimed to analyze clinical characteristics and surveillance rates, revealing that MASLD patients had larger HCC nodules and less frequent screening compared to other groups. Looking ahead, it is crucial to improve surveillance and validate new biomarkers for the early diagnosis of HCC, while also exploring therapeutic strategies that consider the overall health and comorbidities of MASLD patients. Adopting a multidisciplinary approach involving hepatologists, oncologists, radiologists, and specialists in metabolic diseases will be essential for integrated management of MASLD and HCC, ultimately enhancing clinical outcomes and reducing mortality associated with this complex interplay.
