Preparing the hospital for accreditation involved preparing medication safety binders containing a listing of the required organizational practices with hospital policies that support the practice. Presentations were also done to areas of the hospital hosting the hospital accreditors to refresh them on medication safety practices. Unit inspections were performed to ensure no visible medication safety issues. Performing the dangerous abbreviation prescription audit involved reviewing 3 days of prescription orders looking for the use of dangerous abbreviation and then inputting the data into a spreadsheet. Medication reconciliation project involved presentations to introduce the process of medication reconciliation on discharge.
